EMPLOYEE BENEFITS

Community Bank Flex Plan

Community Bank’s benefits package is a flexible plan that allows you to tailor your benefits to better match your situation and lifestyle. We offer medical, dental, vision, employee life insurance, long term disability, accidental death & dismemberment, spouse & dependent life, and health & dependent flexible spending accounts. Community Bank lets you pay for medical, dental, and vision on a pre-tax basis.

Eligibility

You are eligible for benefits if you are a regular employee scheduled to work a minimum of 20 hours per week. Coverage begins following 30 days of employment.

Medical Plans

Community Bank offers two medical plan options: PPO and HMO

Dental Plan

Community Bank offers a PPO/DHMO, which provides comprehensive dental coverage.

Vision Plan

Community Bank offers vision coverage. Exam and lenses are covered every 12 months. Frames are covered every 24 months.

Life Insurance Plans

Community Bank provides, at no cost to you, Basic Employee Life insurance at 1 times your annual salary. You may also purchase additional Voluntary Employee Life of up to 5 times your annual salary. Maximums apply.

Spouse and Child Life insurance are also available.

Long Term Disability Plans

The Bank provides, at no cost to you, the Basic benefit of 50% of monthly earnings. Maximums apply.

Accidental Death & Dismemberment (AD&D)

The Bank offers, at no cost to you, Basic AD&D insurance at 1 times your annual salary. You may also purchase additional Voluntary AD&D. Family AD&D is also available. Maximums apply.

Health & Dependent Care Flexible Spending Accounts

Health & Dependent care flexible spending accounts are available.

Employee Assistance Program (EAP)

An employee assistance program (EAP) is available for all employees. This program provides confidential professional assistance to help employees and their families resolve problems that affect their personal lives or job performance.

401(k) Retirement/Savings Plan

All regular Community Bank employees are eligible to participate in the 401(k) Plan. You may enroll in the Plan at the beginning of the calendar quarter after completing 90 days of service, if you worked at least 250 hours during that period. The Bank will match 100% of the first $150 you contribute plus 50% of the first 6% of your compensation that you defer.

Profit Sharing Plan

The Bank may also make a discretionary Profit Sharing contribution to the Plan, based on year-end financial performance.

Vacation

All regular Community Bank employees working 20 or more hours per week accrue vacation from the date of hire. Employees are eligible to take vacation after completion of their 90-day introductory period.

Bank Holidays

Community Bank observes ten established federal holidays.

Sick Pay Plan

All regular Community Bank Employees working 20 or more hours per week accrue sick time from the date of hire. The accrued sick time can be used in conjunction with disability benefits when employees are absent from work for an illness or injury.

Educational Reimbursement

Community Bank will provide tuition assistance for employees to attend seminars, workshops or conferences related to their job. The Bank will also provide reimbursement for college courses taken (maximum payment of $5,000 per year), after successful completion of pre-approved courses that relate to the employee’s present position or potential development in the Bank, or lead to a degree or certificate in fields that are business related.

Free Bank Services

The following services are offered free of charge to Community Bank employees: Checking Account, Savings Account, small safe deposit box, online banking, entertainment discount coupons, and a Visa Card with no annual fee, provided you qualify.
